MS5 is a Portland-based parent organization. We operate multiple community programs — currently including MS5 HRSN, MS5 Wellness, and MS5 Preschool — each running as its own program with its own team, all under the MS5 umbrella and sharing a single office.
MS5 HRSN, our Health-Related Social Needs program, offers rental and utility assistance to Oregon Health Plan Open Card members. To date, MS5 has issued more than $686,000 in direct assistance to families across Oregon through this program.
MS5 Wellness and MS5 Preschool extend the organization into outpatient behavioral health and early childhood education.

All programs operate from a single office in Portland. Members can be referred between programs without restarting paperwork.
